MP4 files typically store their audio as AAC, so the track is re-encoded to MP3 at 128 kbps for stereo sound and 64 kbps for mono, such as most phone and meeting recordings. For speech that is plenty. For music from a high-quality source, a careful listener may hear the extra compression, and MP4 to WAV keeps the audio uncompressed instead.

What's the maximum file size?+

Will quality drop?+
A little, in principle. MP4 audio is usually AAC and MP3 is a different codec, so the track is always re-encoded: 128 kbps for stereo and 64 kbps for mono. That is comfortably enough for speech. If you want the audio with no further compression, MP4 to WAV writes it uncompressed.
